Referring for implantology
You remain the lead practitioner. We carry out the implantological part that we agree with you beforehand. Afterwards you receive a report and your patient returns to you.
What you can expect from us
A clearly defined assignment
You decide what we do: only the surgical part, or the suprastructure as well. We record that beforehand.
Your patient returns to you
After the agreed part we refer back. We do not approach referred patients for regular dental care.
A usable report
You receive a compact report with imaging, the components used and maintenance advice.
From referral to report
The patient remains free in his or her choice of care. Our way of working is aimed at collaboration with you as the referring dentist.
You submit the referral
We contact you and agree how you deliver the diagnostics and your question securely.
You receive a confirmation
With an indicative planning date within the agreed period.
Intake and treatment plan
After the intake we report the treatment plan and estimate back to you and your patient.
Treatment
We carry out the agreed part. If the situation deviates from the plan, we consult you first.
Report and referral back
You receive a report, photos, X-ray images and maintenance advice. The patient returns to you.

No patient details in this form
We only receive the patient's name, date of birth, BSN, X-rays and medical details through a secure channel, in line with the GDPR and NEN 7510.
